Participant list yet anonymous results
I would like to create a quiz (open only to those in my organization), that will anonymously collect the response (simple "agree" or "disagree") but ALSO give me the collective emails or names of who...
jmmogg that's not do-able. Either the form is anonymous in which case there is no way (as there shouldn't be) to collect details of who submitted it. If in the settings you select that anyone within your organisation can complete it then their email address will automatically be collected and linked to their response.
