Multiple Text Boxes or Multiple Lines

Hello,

 

 We have a question that asks for name, company, email, and phone for entry into a drawing/raffle.  In another survey software this can be done with multiple text boxes on one question. Is that possible in Forms Pro?

 

Contact Info -- would be the title of the question and these would be the text boxes:

    Name 

    Company

    Email 

    Phone

    Hi Beth JonesEA_2018  you'd really need to have one question for each piece of information, otherwise you can't ensure that the responder has completed all of them. We did this for a raffle across our company a couple of weeks back and saved the responses to a Sharepoint list via a flow in Power Automate, and then had a second flow that ran once to select a random response (the winner) from the list. That was fun.

      I just revised the question to an option to click "Yes" to enter the drawing since we already have at least their email address. 

       

      Also, thanks for the idea on how to pick a winner!

      Hello, Pl share how can you add multiple box in MS forms in one question like:

      For Postal Address :

       

      Address Line 1

      Address Line 2

      Area

      City

      State

      Pin Code

        Chandresh243 you can't, it's not a feature in Microsoft Forms. You will need 1 question for each of those pieces of information.
