Multiple responses plus text box

Hi all, 

quite new to forms so just finding my way. 

Is there a way to collect multiple responses (tick boxes) but then to also add in a value beside those selected boxes?

    c_fitzmaurice no, unfortunately there is no way to do that. I think this is covered by the uservoice request at https://microsoftforms.uservoice.com/forums/386451-welcome-to-microsoft-forms-suggestion-box/suggestions/37219639-create-matrix-question-with-text-box which you can vote for.
