MS Form Icon on Mobile Device - How to change it?

Hi Team, 

I created MS Form which can is available to be accessed via desktop and tablet / mobile device. 

When the Form is downloaded to the phone it comes up as a very generic icon - how do I change this so it has an image associated with the form?
