MS Form fails to load on desktop version of MS Teams

Hi,

I have a MS form embedded into a MS Teams channel as a tab, but this has been intermittently failing to load over the last few weeks. 

It feels as if 80% of the time the Form fails to load.

When clicking the MS Forms tab, the loading symbol appears briefly, followed by a blank screen. This appears to be working within the web version of MS Teams, but our department prefer to use the desktop version, so would like to raise this for further investigation.
