MS Form auto-generate new blank section

I encountered a weird problem in my MS Form. It always auto-generate a new blank, unwanted section at the top of the form whenever I refresh the page, edit the form or any other action. Is it something goes wrong with my form setting or is it a bug in MS form?

In my case, I have already created 45 sections for decision-making paths
