Missing Folder In One Drive For Upload File Function In Microsoft Form

Hi, If I created a form for internal use and have set up 2 upload file functions in the Microsoft Form.

Currently, I am unable to find one of the documents which stores the upload file in One Drive.
Even I go to the Form > Responses > View In Folder, the forms weren't able to lead to One Drive.

When I check in One Drive, the folder already missing and the Microsoft Form is now unable to upload files without a folder.

I tried to create back a new folder with the same name but it didn't work. 

May I know any solution than create a new form?
