Microsoft Forms save files in OneDrive - change file name
Files that are uploaded in a Forms survey are saved in OneDrive. The name of the person who completed the Forms survey is added to the file name. How can I change this so that the file name remains unchanged?

Microsoft Forms automatically adds the uploader's name to the file name when a file is uploaded. There is no built-in option to change this.
You can post your issue as a feature request on Microsoft Feedback Portal:
Microsoft Forms · Community
However, you can also try to modify the file name using Microsoft Power Automate after the upload.
