Microsoft Forms Response Page Blank

Since Sept 25th the response page of my forms is blank. The response page is still blank when I use Google chrome (mobile phone and desktop), also Microsoft edge in desktop, I've tried to clear the cache but still not works.

Meanwhile, In mobile phone google chrome incognito mode it works, on opera mini also works.
