Microsoft Forms Reliability Issue

Hi, I created a form for a charity survey and am going into review the responses. Its a relatively short survey and we have had a few responses. However I'm not able to access this form any longer and keep getting this. I noticed that something similar had happened to a number of users in Feb. Is there an issue with Forms and pease advise how I do a recovery of this form and the responses.
