Lost Microsoft Form

Hi everyone, I created my first form and successfully began to receive responses. A couple of weeks later and the form seems to have vanished. I really need to access the form. 

I've tried replacing "ResponsePage.aspx?id" to "DesignPage.aspx#FormId" but only other forms are visible. It's not in deleted nor pinned. 

I really need the information ASAP. I'll be grateful of any help.
