Issue with Form automatically duplicating

We have a form that triggers a flow for an automated process. However, the form submissions are automatically duplicating themselves.  There seems to be a 5 second delay, but it is causing major issues by sending approvals multiple times.  The issue also seems to be growing impacting more forms and more duplications per form.

Any advice is greatly appreciated.
