Invalid survey url during form submission

Hello community experts,

I have created a form, like below

And it throws me this error when i receive a mail and try to submit the form

My actual requirement is "once a case is resolved, send a mail to the customer and get the feedback"

I'm struggling a lot, it is an urgent requirement, please guide
