How to design a form with data based on SQL Server?

Hi All,

 

Can we create a Microsoft form with data based on SQL Server? If yes please let me know.

    skumar it's not possible as Forms has no ability to look up data from anywhere.

        skumar you could build it in Power Apps which can use SQL Server as a data source.
