How to create a Form that allows end user to submit multiple entries in a single submission

Hi all! 

I am trying to build a Quality Issue Documentation form where the end user, a Quality Assurance team member, can fill out and submit issues discovered on a given product.

The form will collect basic product information like type of product and serial number. But what if the same product of serial number XXXXXX had multiple issues? Is there a way for me to set up the form so that the user does not have to re-enter basic information like serial number for the same product and to only enter the additional issue(s) information?

The output in excel I want is:
Product Receive Date | Product Serial Number | Issue Description | Affected System/Subsystem/Part
12/12/2012 | AAAAAA | Train horn malfunction | Horn
12/12/2012 | AAAAAA | Scratches on train car exterior | Body Shell

FYI, above is a fake example. 

Can I set up form in such a way where I can get the above output in excel?

    Isle0fCats What you want to do is not possible as Forms cannot pre-populate data in the questions from Excel or any another datasource or from previous questions in the same form. So the user will need to re-enter the basic information each time they complete the form.
