How to allow the user to add text boxes in microsoft forms ?

I would like a create a form where the user can fill in their skill set/ competency and rate themself . I would also like the user to add any number of skill sets with an option " Add new" as shown in the attachment. Is it possible in Microsoft forms?

    ANUSHA1091 it's not possible in Forms to have the user enter likert statements in the way you have shown. It would need a form built with Power Apps instead.
