Getting started with Forms Pro?

HI Folks, 

I'm trying to get started creating Forms with Forms Pro.  My user has a Forms Pro license via D365 CE Enterprise license, also have the Forms Pro Trial license assigned to my user profile. 

How ever, when I try to create a Form in https://forms.office.com I only see the option to create a classic Forms Survey ..

what am I missing in the setup?
