Forms to OneDrive Error

I have created a One Drive form. For a while, it worked as it should (when an entry was made to the form, it showed up accurately in the accompanying spreadsheet).  Yesterday, however, when anyone made an entry to the form, their response was replicated repeatedly in the spreadsheet. For the 10 entries to the form, I had over 3,000 responses in the spreadsheet. 

 

How do I stop this from happening? I've built out the spreadsheet quite a bit, so I would hate to have to delete and start over if there's another solution.
