Forms submissions not registering

We have an open form being completed by clients. Largely this is working as expected however there are a few users where they are making a submission, seeing a thank you message, and the responses are not being captured. They are not showing in either the form results data or a SharePoint list that is being fed by a power automate.

Can anyone advise of any factors that may cause this to happen?
