Forms multiple choice with amount

If you mean adding it to the form itself when the user makes the choice so that they can see it, then no you cannot.
Depending on your scenario, you could have branching questions to suit the path of the form if a particular option is selected, but I don't know enough about your scenario to give you a more accurate set of steps.

This is very poor. Believe allowing user to select an item, such as a Product and then Choose Quantity is basic functionality for forms in general.
