Forms Data Not Recording Correctly

I have been using forms to track time requests from my employees. The form was working properly until today when it added a random "other" result. It is now tracking responses based on login credentials instead of the actual questions on the form. It is throwing off my data. Someone, please help! Redoing the form is not an option!!!!

        Hi Bianca

        Not sure what's happening to your form with this one. If you are using the Choice option, I also note that there's no radio button to choose a name which looks different to mine which does have one.

        Have you tried making a copy of the form to see if that resolves the issue?
