Forms crashes when trying to collect answers

User Impact: Users may be experiencing unexpected crashes or freezing when using the Microsoft Forms service.
More impact: Users may also experience the issue when attempting to click the “Collect Results” button.
Current status: Our review of service monitoring telemetry has so far proven inconclusive. We’re working with affected users to gather reproductions of the issue, to further the investigation into the root cause.
Scope of impact: Impact is specific to some users who are served through the affected infrastructure.
