Hello there. It’s been a long time since your request, so I hope you’ve already found the right solution. Still, if I were you, I’d hire someone on Fiverr. Fiverr specialists could give you even more than a simple form. Actually, I have a church of love near me. They have a pretty good website and an excellent form, so I could ask them for help with your issue if you still need it 3 years later… I wish I had seen this thread earlier. Anyway, I hope I can help you or someone else. I’ll be waiting for the replies.
