Error on MS Form : form can't be distributed as it is asking for personal or sensitive information

I have a form to collect member details for members at my place of worship.

The form is not currently accessible with following error "This form can't be distributed as it is asking for personal or sensitive"

It doesn't ask for username, passwords or financial information of any kind.

We are just collecting name, contact details, education details and blood group. Blood group if just to create a database for use incase of emergency.
