Hello Colzer, I have the same problem : my customer has a lot of forms, and the QR codes are obsolete. I'm asking help to the ms hotline.
Did you manage to solve this. I have the same issue. We have used the QR code on marketing material so would really like to know why it's doing this all of a sudden
