Cannot access form responses after subscription renewal

Help needed. I created a Microsoft Form before Christmas, link below.  I had to renew my subscription and now the form does not come up on the Forms dashboard so I cannot access the responses

I have about 3 365 logins - one personal, one work and an institutional one but cannot find it on any of them! Please help
