Matt PICKLES no unfortunately you can't add a comments box to a form. There are lots of different uservoice requests for this at https://microsoftforms.uservoice.com/forums/386451-welcome-to-microsoft-forms-suggestion-box?query=comments%20box

This seems like a very basic feature for surveys. Why isn't it available on Forms. The branching feature is not even a band aid, more liking putting a kitchen towel on your severed arm.
It is really difficult to comprehend why Forms does not include such a critical feature.
Simple example:
What features would you like to see?
Feature A
Feature B
Open Text Box
Looking forward to positive news.
