Can a file be embedded at the top of the Form for respondents to use while completing the Form?

Is there a way that a file can be embedded for respondents to use while completing the Form? We answer questions about calls and it would be helpful to upload a file with all of the call information from SharePoint instead of having to have the respondents go to two locations to complete the Form.
