'Broken' Forms Icon on MS Edge [+New Tab] App Launcher

Hi, I'm in an education tenant; using MS Edge in Office 365 Mode.
When launching a New Tab and selecting the [App Launcher] in the top left, of the 12 default O365 Apps displayed, the Forms Icon is broken.

I am hoping others can check their tenant to see how widespread this issue is, I have checked and identified a second tenant in AU.  Screenshot attached.
