Branching GLitch

I believe I have a bug in the branching of my form (on forms pro). If a particular option is selected on any of 3 questions, 4 new questions should become visible. This only works for 2 out of the 3 questions. I have tried re-ordering the branching rules, re-ordering the questions. No matter what, I can only get the same two conditions to show the follow-up questions.
