anyone else getting this error 631 message?

my problem also the same.
even when I want to create the new form the same error come out.
help me to fix this.

I get this 1003 error, when opening forms form starting menu. I also have a teampage in which a form is added which gives the 631 error.
Now when I open Forms from the menu button in upper menu I got a "Loading" icon, that doesnot stop:
