All questions on a Form have disappeared.

I developed a 21 question Form (Office 365 for the web). I obtained the URL by clicking the SHARE button, and pasted the URL into an email.  People receiving the email immediately contacted me to tell me there were no questions in the form.

Please tell me I have not lost an afternoon of work and I have to rebuild it...
