Adding client identifiers on each page

I work with assessment forms for allied health and nursing staff. We are looking at producing these forms via Microsoft forms. I have had no trouble with customising the form but as a legal requirement, there has to be 2 client identifiers on each page when they are put in the client file. Does anyone know how to do this? I am new to this so my apologies if this has already been asked. The identifiers woudl be name and DOB
