How to add an new fillable file/field/folder on demand

I created a fillable word document which requires the user to complete certain fields based on observations. The event is not a singular one and might be repeated from 1 to 50 times.

How can I get the user to add a new blank document every time he/she encounter a new event?

I am looking to add a question at the end of the document that would ask - New Event? Yes / No

By choosing Yes, it would automatically add to the existing document, a new blank version of the original and by clicking No or skipping the answer it would stop.
