Have you checked the templates for Flow at https://emea.flow.microsoft.com/en-us/templates/ and searching 'Form'. There are several pre built templates, one may be suitable although its likely that you'd get an email for every response so may then need to tweak the Flow to ensure it only notifies when 100%.
