Tracking progression in excel with different IF conditions

Hi,

Can someone tell me if this kind of conditions can be done in a formula? As a novice in excel, I've been stumped on this for the longest time. Hoping someone can give me some insight on making this possible. I appreciate the help!

The conditions are:
1=Fail / 0 = Pass

1. If the employee fails the metric once in a 3 month look back period, the cell value is "Warn"
2. If the employee fails the metric more than once in a 3 month look back period, cell value is "Enroll"
3. Once an employee is enrolled, if he fails the metric within a 6 month look back period, the cell value is "Progress"
3. If the employee was recently enrolled or had progressed, but was able to clear the 6 month look back period, the cell value is "Cleared"
