Revenue Forcast tracking workbook

Hello, Earlier this year I found a revenue tracking workbook on excel that I modified for my needs at the time. 

I am now wanting to use this workbook and I can not for the life of me find it. 
If anyone has a copy of it or a similar one are you able to provide it for me? I am not excel savvy and need a revenue tracking workbook.
