OCR with entity extraction to Excel sheet

I need to convert thousands of physical index cards with client information on them to a Excel sheet. 

The cards contain a vast amount of unnecessary data, thus I need to identify the 3 specific entities I need (name, address, city). 

The cards are very roughly formatted but the data I need is generally in a similar spot, give or take.

The data needs to be recognized then placed into a multi-column sheet (e.g. for name, address, and city).

Would there be anything that could reliably do this? I’m open to using AI and whatnot provided it is free.
