Let's say Date Entered is in column D, Date Received in column E, Date Started in column F and (Date Completed + Operator) in column G, starting in row 2.
Enter the following formula where you want to see the status:
=IFS(G2<>"", "Completed", F2<>"", "Started", E2<>"", "Received", D2<>", "Requested", TRUE, "")
Fill down.
