deuce616 The form in your file is protected, so you can't do much with it if you can't unprotect it. But you can copy the entire sheet to a new one and copy the Commission's logo in to it. Then you can add Data validation to the Permit No field and use INDEX an MATCH to fill in the relevant information from the data sheet into the Form. Started doing it in the attached file, but gladly leave the rest to you, in case you decide that this indeed is what you want/need.
