makeing a form to which i can add controls and load single rows into a table

to be much more concise, i need to know how to create a form to which i can add controls.. the form would be used to load single rows to an excel table, which will calculate certain fields and ideally the form would update those fields in real tome while the user is filling in the form's writable fields....
