If you are lucky to have Excel with dynamic arrays, formula here
in B2 could be
=XLOOKUP($A$2:$A$49,Sheet3!$A$2:$A$151,XLOOKUP(B$1,Sheet3!$C$1:$F$1,Sheet3!$C$2:$F$151))and drag it to the right.

:You say "It's a separate sheet" but don't really respond to Abiola1's suggestion of INDEX/MATCH or to mine of VLOOKUP (the two suggestions are very similar for the most part). I'm sure both he and I would appreciate a more complete response.
