How can I use index match if it wasn't in one column?

My bad. Yes! I want to create another sheet which display the PRN, NAME, POSTN and the specific cell value whenever I choose any number from F11:AJ31).

- I am still confused with this, you do not have data up to AJ31 did you mean AJ11.
- I just didn't fill it up. All I want is to make a cell that will display like what in the second picture.
