Help with trending data

Hello, I was wondering if anyone could help explain how I could show in a line graph trending data for 11 weeks worth of data. The data: there are 5 questions asked over 11 weeks with answers being Yes, no, N/A. I would like to see the changes in answers over the 11 weeks. Thank you
