Do you have Insert -> Form available on ribbon of your Excel for web?

No, I don't have FORMS as an option to insert:

  If so most probably it's not available for your subscription. I'm not familiar with G5, the only was able to find this comment

    Thank you Sergei - you may have hit the nail on the head with this snip from the URL:
    We do a lot with Forms, but the "Forms for Excel" web add-ins probably fall under this "Office integration" category 😞
