Hi Emma,
You can use a formula such as this one:
=LOWER(CONCATENATE(IF(Starting_Part="First Name",A2,B2),
Separator,
IF(Starting_Part="First Name",B2,A2),
"@"&Email_Provider&".com"))
To make the formula dynamic, I've used some options in the right table and made the formula refer to them.
