Has anyone been able to contact Microsoft to try and talk with someone about the process and find out details of the holdup?

Unfortunately, I'm unable to troubleshoot the status of your application.
My suggestion is that you review the direct bill steps to ensure that you've completed each step; otherwise, you will need to submit a support ticket & select "billing" as your category:
