As far as I know, the per app license will only cover one app in one environment for one user - https://learn.microsoft.com/en-us/power-platform/admin/about-powerapps-perapp
If the same app or website exists in two environments, a user will require two per app User SLs to access both.
My understanding is that once the Power apps per app license is assigned to an app to an environment, it remains assigned to that app/environment for at least 90 days.
