Let me try to explain what I am trying to do: I would like to have a single question where I have a list of items showcased and the answer I would require is a numerical value that is assigned to the listed item. In the sense that I would like to ask a percentage of a feedstock material that a facility receives and would give 3-4 common options and would like for the people to answer a numerical value corresponding to the showcased predefined answer. I have searched and not found that this is possible with Microsoft forms, even that I think should be relatively simple from the code perspective. Multiple choice answers only accept select the box option and a numerical value is only available in text answers. The not attractive solution I have found is have 5 questions one behind the other that each accepts a numerical value answer, but this is not the most stylistic and clean solution. Anytime someone external submits a MS Form, it shows as anonymous in the email notification. There needs to be an option to show the name or first name, last name field on the form or have the option to send a notification with a summary of the individual responses. This is helpful for when I'm collecting responses for job application candidates across multiple different roles/forms within a company. New to forms, trying to create a survey that users in my organization can use to indicate if they're using software. When I copy the URL and send it out, I can access the form myself as the form creator from the link, but everyone else that tries to access the form gets the error message in the title. The technical details are: I've had the users try a different browser with no change. I have it set so that only people in my organization can respond as I want to record names. Not sure where else to look for access permissions. I've read many of the threads with the same error message and have validated that all settings should be correct for access and that the people I'm sending the form to are logged in with their business account. Dear sir, Company domain: catensys.com We use Microsoft enterprise platform, we found one issue today: When we submit form answer, it do not send data to sheet. Form record list 4, but sheet only have 3 rows. And sheet display note: 10% synchronized. This workbook is checking for any changes from Microsoft Forms. Please support us to solve this issue. Hello Have created a group form and would like to use this form as a template for different events. Excel spreadsheet with responses is used with links and formulas in other worksheets. How can this spreadsheet containing the formulas be used as a template to use with other forms ie trying to avoid having to recreate the other worksheets that contain the formulas. Have tried various workaround which don't work: 1 Duplicate form / delete responses = does not delete responses from spreadsheet 2 Copy worksheet to new spreadsheet = does not work as the spreadsheet filename is part of the formula, so refers to the original spreadsheet (have tried find/replace all, but very cumbersome) Any suggestions, would be greatly appreciated. A week ago when I created two new forms and both not allow me to collect responses and has this message "This form can't be distributed as it is asking for personal or sensitive information. Terms of use" with the collect responses button greyed out. The forms do have a request for names, date of birth, address, telephone numbers and email addresses. Hello. I have created a Power automate to capture the response when a form is submitted. We allow the users to edit the from after submission, sometimes they don't have all the info at first submission. Does anyone know how to capture the resubmission of the form after the user edits it. The Excel sheet that the form is linked to does get updated with the resubmission but I cannot use this to trigger an alert for the edit. I am trying to avoid linking the form to Sharepoint list just to capture the change. Ideally I would like to trigger a workflow directly from the resubmission of the form.
