Userform HELP

Greetings Everyone,

Need your expertise and assistance please.  I am fairly new to VBA so please bear with me any growing pains if I ask too many questions as I'm learning.  I understand that the only way to learn is to take a stab at it but I'm stuck. 

Anyways, I would like to create a userform that a user enters the information and within the body of the letter, I would like the user to choose 1 of 3 predefined text and insert the text in the document once the button is selected and upon clicking on the submit button, the userform produces the Word document with the correct language.  Sample attached.
